Sign up for Services
To receive services (have the bill put in your name):

If you are:
The owner of record (as shown by the Allen County Auditor)
A company representative (provided by the owner)
A recent buyer (deed stamped by the Recorder’s office)

You will need: A completed Utility Service Contract and a picture I.D.

This means you will be able to request all extension/payment agreements in accordance with Utilities Rules and
Regulations, and you will be permitted to request in writing any leak adjustments, pool credits, refuse adjustments, and sign
releases to have service restored in accordance with Utilities Rules and Regulations.

If you are:
Other than owner of record (Tenant/ Land Contract holder/Realtor/Bank/Rent-to-Own)

You will need: Authorization form completed by the Owner or Company Representative on file with the Customer
Service office, plus a picture I.D. (Forms can be faxed, e-mailed, or hand delivered)

A deposit of $150 (nontransferable)
Utility Service Contract form signed and a picture ID required.

If you have had services shut off for delinquency:
Tenant/s will be considered high risk and will have their deposit raised to $250. This will be
charged in increments of $50 monthly until the $250 deposit is achieved. (Applies to single
residential units only.)

Written permission from owner or company representative is REQUIRED PRIOR TO all requests for
extension/payment agreements in accordance with the Utilities Rules and Regulations.

You WILL be permitted to request in writing, leak adjustments, pool credits, refuse adjustments and
sign releases to have water restored in accordance with the Utilities Rules and Regulations
